Wellness Program
The 2024 wellness program, provided by Telligen, aims to improve the health and quality of life for City of Chicago members and their families. Telligen programs may be mandatory or voluntary depending on your health plan. The Telligen team collaborates with you and your care partners to provide the best possible outcomes.
Disease Management Coaching
Telligen offers chronic condition coaching for members who have been diagnosed with a chronic condition as well as metabolic syndrome/at-risk coaching for those who have elevated risk factors. Telligen nurse coaches help members manage their conditions and mitigate health risks.

Utilization Management
As required by the City’s healthcare plan, the goal of our Utilization Management (UM) program is to provide a fair, evidence-based review of the care you are receiving to determine medical necessity. The UM program protects you from receiving treatments that do not meet the standard of care.

Case Management
Following a health crisis or a new diagnosis, Telligen’s nurse case managers engage members in a collaborative process of health assessments, planning, care coordination, education, and advocacy of care. This is all done with the final goal of members having the ability to successfully selfmanage their care and condition.
