Hospital Showcase Form
The Medicare Beneficiary Quality Improvement Program (MBQIP) Hospital Showcase Form is a comprehensive tool designed for critical access hospitals (CAHs) to document and share their quality improvement work with Flex coordinators and peers. Flex coordinators can use this tool with CAHs to capture valuable information to share with Flex participants. This structured questionnaire guides CAH teams through all phases of the PDSA (Plan-Do-Study-Act) improvement cycle, capturing problem identification, intervention testing, results analysis, and lessons learned—regardless of whether the project succeeded or required adaptation. By completing this form, CAHs share valuable knowledge with the rural healthcare community and show their commitment to quality improvement to stakeholders, boards, and patients.
