During the 12th Statement of Work (SOW) QIN-QIO work that ran from 2019 to 2024, Telligen led a network of partners working on healthcare quality improvement initiatives that are data-driven and locally tailored to improve healthcare quality and outcomes by implementing and spreading evidence-based and best practices. This network included 3,278 providers: 1,795 NHs, 144 Critical Access Hospitals, 33 rural IPPS hospitals, 10 tribal hospitals, and 3 rural emergency hospitals. Telligen’s contributions had the following impact:

  • C.diff Hospitalizations: Reduced by 25.1%
  • All-Cause Patient Harm: Reduced by 14%

Hospitalizations & Readmissions

  • Medicare 30-Day Readmissions: Reduced by 18.2%
  • Avoidable Hospitalizations: Reduced by 12.5%
  • All-Cause Readmissions: Reduced by 20%

Chronic Disease Management

  • Uncontrolled A1c Levels: Reduced by 37%
  • Missing A1c Level Documentation: Reduced by 60%
